A match manually ? I remember when I was in high school a long time ago, my coach had a chart, but I forgot what all was on there. Let me know if anyone has a link to a chart or something like that. Thanks.

yea we used to use these until my coach got a program on her PDA. i dont have a nice form but i know what was on it.
ok so its got 3 large columns: Player 1, Score, Player 2
each player column is divided into 3 smaller columns: winners, errors, forced errors.
so it should look like this:

sorry its messy (made in paint) so you can make a better one in word or something.
now for using it
score:
each score box is for one game only. at the end of the game, you record the score and move down to the next row. example: player 1 wins the first game. the first row score box is 1-0, then start charting the next game in the next row.
Shots:
you only record the point-winning shot. example: player 1 hits a backhand winner. on player 1's Winner section, you mark BH, etc. this works for BH FH aces, lobs, double faults, everything. same goes for errors and forced errors.
add up the shots at the end of the match and you're done.
hope thats the kind you were looking for. theres also some that just count up total numbers of winners etc, but this is probably the easiest and most in-depth one ive used.
