These are all forehand grips.
Backhand grips are Continental, Eastern, Semiwestern.
Eastern Bh is index finger base knuckle on the top of the handle. It is what you would hold if you truned your Western FH grip over without moving yoour hand on the racquet. Western FH is on the bottom, Eastern BH is on the top.

Eastern backhand and continental are the most common. I personally use eastern backhand because extra spin and kick on the serve.

I was struggling to get much kick off my serve. I haven't had a chance to get coaching on it and I think my technique is wrong but I switched to an eastern backhand from continental and all of a sudden I was able to get it kicking fairly well.

Wrong...thats continental.
First go to continental. Then move your hand to the left so the v of your hand is on the next corner.

um.. no..
what the 2 post front of me guy described is eastern.
you put your index bone(?) on top panel and that's eastern backhand grip.
and you move one to the right. (1 oclock) and that's continental.

Are kick serves particularly stressful for the back or shoulder? My back can be aggravated from flat and slice serves as it is, and I would guess kick serves might even be worse for it. Anyone know?

If your back hurts on regular servers, a kick serve will not help. My back is fine, but whenever I kick it on the ad court (I'm left handed) and I want to kick it to the fence, it kills my back.
