Grommets are there to protect the string from the sharp edges of the racquet. So, if they are beat up enough where, they can't do that any more, it's probably time to replace them. Until it get's bad, it's a judgement call on your part. You can also work around bad grommets by using nylon tubing.
It's probably more common for the plastic head guards to wear out from court abrasion. Since you typically buy the grommets along with the head guard as one, when the head guard gets replaced, you typically replace the grommets. Again it's a judgement call as to when the head guard needs to be replaced. Some people like their racquets looking pristine and will replace them frequently, while others don't bother and just let their racquets get beat up.
