Popping string at 12

My coach borrowed my freshly strung Ashaway Monogut 17 and took a serve with it. He popped it @ 12 o'clock with one serve. How does that happen? This happened just minutes after he popped his last frame @ 12.

Maybe it's your grommets. If they are new, I highly recommend that you put tubing in the hole right above the part where the string broke.
