Anyone here absolutely never have to re-toss because they botch a toss? I don't think I have had to re-toss a ball since some time in 2006. Not that my tosses are always good, but I always swing at them rather than try again and they've never been so bad that I was unable to hit the ball.

I am in the same boat. My tosses are generally good and as a policy i dont retoss. The only problem i occasionally encounter is with the height of toss than the placement of toss.

Is there any other problem that can occur with the toss besides these two?

The toss is a major problem for me. I think it has to do more with coordinating the toss arm with the racquet arm, because just tossing by itself I don't find to be a problem. It's when I think about the racquet arm as well.

My toss is not consistent so I re-toss probably ~3 times during my service pt.

My toss is terrrrrrrrrible.
gah.
My serve is good, when my toss is on.
I usually have to toss a couple times for a good one.
It's frustrating, but very vital to a good serve.

Eh once in a while maybe couple of times during a set I have to re do the toss...

But things change drastically when I'm playing in the wind... I hate winds.

If it leaves my hand, I hit it. I don't think I've ever re-tossed the ball. I'm not saying all my tosses have been great, but they've all been hittable.

I re-toss a lot. My main toss problem seems to be tossing too far to the left.

I wish I would re-toss more. Most of my faults come from hitting bad tosses. I think the same thing every time: "Oh, go on. Hit it."

Your so right.

I can practice my toss all day long and it's perfect, but when I go up to hit a serve it becomes a lot more complicated.

I will occasionally catch my toss merely to throw off my opponent - never because I actually need to.

Seems like you aren't maximizing your chances to hit good serves. Hitting a mediocre toss probably means the serve will be mediocre. Or at least not as good as it could be.
