Road test paperwork [Archive]

So my roadtest is on monday. I have all of the paper work that i need except for the 20 hour driving form.... Does anyone know a place to print it out online? I looked at the DMV website but no luck. It was too confusing. Would writing on a piece of paper that i have driven 20 hours and have my mom sign it work?

I am from NY thank you... To bad i cannot download it. PDF's don't work with my CPU i guess

Could anyone be nice enough to put this on theri photo bucket and post it here so i can get it =D
