Close as I can come is:
Method of payment
Bar(?)/Cash on delivery (+1.50 Euros)
Calculation
Direct debit
Method of shipment
Looks like the rest is the address for "Evtl. Company"

I would agree with your translations (Sepp :-)) and....
Bar means paying in cash
Rechnung means both receipt and bill, as well as calculation -
in this context I think though it means they will include a bill with the shipment which you then pay later (probably only available in Europe (Germany/Austria ?))
Evtl. means (in this context) if applicable (i.e. the name of the company if you are one)
Anrede is title (but Name (last name) seems to be missing)
Vorname - given name
Strasse Nr - street number
PLZ is zip code
Ort is town (in an address)
