. . . I haven't. Even though I use a 2-pt. mount machine. But have you had this happen. . . and why? what caused it?

I cracked my frame 5 times all on the SERVE when it sliped out of my hand and pronated right into the ground at 100 MPH.

Yes, I cracket my own PK Composite Dominator the first year they came out (mid 80's). I strung the crosses from the bottom to top. It cracked at 10 o'clock. Called PK. They explained how to string an ATW pattern, and replaced my racket. Haven't cracked one since, and I've done a bunch.

I have never cracked one. I have been stringing since 1975, I have strung a few that I told the person, that it might not make it, but they did. During the old Prince Pro days, you could see the racket bend during the stringing of the mains...

Yes, I used to string my Prince Pros @ 80 lbs. (Rec tension was 72 lbs.) This was in the days before racquets had bumper guards too. The aluminum was really scraped up and I got bilateral fractures along the rim of the racquet as I tensioned the mains.

Recently a pair of Princes More series cracked @ 9 o'clock while in the machine when pulling the tension<57LBS> on a crank machine.

Like LtlElvis, I've strung Prince Pro's at 80# also. They actually recommend stringing at 80# for more "Control"--as printed on the side of the racket. I'd really worry about the string at those tensions, but when you see those frames distort, you worried more about the frame.
