It's a foot activated drop weight. Very similar to a Serrano.

Oh gosh! I see that's a 'wing nut' twist on the clamp. Yikes! It'll work, but there's a reason we don't see that kind of clamp on current, quality machines. It'll do WONDERS for tennis elbow!

The limitation would likely be the mounting hardware as it may not work for head size over 90". And the clamps could be very slow but you may be able to get different ones?
I just got a Serrano and love it but it has upgraded mounts and clamps so it will work for modern frames. So this could be worth it if fully functional and if it will mount modern size frames.
