It is difficult to say. It depends on what is being done and how successful it is being done. Then it depends on complications, caused by the patient's physiology etc.

Reoccuring of the same problem is not abnormal.

It is always advisable to try everything, before going into surgery, but, I suspect, that everything outside of surgery has already been tried. And, yes, it will offer big improvement, if everything goes well. It will take a lot of time, though. Returning for AO2013 might or might not be possible, depending on how the treatment goes, but he can probably rely on the best doctors in the world, so a good chance for him to have perfect conditions to try to return for the AO.

I think Nadal's condition is best treated by his own physicians rather than the residents around here.

Just guessing ... maybe Rafa does not want to decline DC, he would rather say Yes, and then the Spanish Federation say that he should rest his knees. That way he skips DC but no one can say he declined.
