Plenty of times, at least where the aces are concerned. I've seen Roddick serve four consecutive aces several times, Federer at least a couple, and Querrey once had two consecutive service games each consisting of four aces in a match against James Blake.

Sam Querrey holds the record for most consecutive service aces since the ATP began keeping records of those statistics, with ten.
 
I've seen Coria lose his serve only by making consecutive double faults. I guess Verdasco also made 4 DFs in a row versus Nadal recently when he was winning that second set by 4-0 (he then lost it, 7-5).

Greg Rusedski hit 5 double faults in a row against Mikhail Youzhny at the 2006 ATP Masters Series Miami. He went on to win the match 6-3 6-1.
He trailed 3-1 after three double faults in his first service game and two more in his second.

Becker once served 5 aces in a row at 0-40.

I recall that, it was indoors. I know Sampras served 4 aces in a row to open his 1998 Wimbledon match vs Enqvist, and 3 to open his 2nd service game, so he won the 1st 7 points on serve with aces.

i have the following one... let's call it "4.5 DF" ! :p
ivanisevic, against caratti in the US open 1999, started a service game with 3 DFS... makes a 4th one to complete the thing, but caratti then ruins all his efforts by calling his last serve good !
no problem for goran, who serves a "5th" one to properly finish the work. ;)

Isner just did it last night at a US Open/Emirates Air series match in Atlanta. He was up 6-5 in the third set. They opened up new balls......and bam..... Isner dropped 4 aces in a row to win the match over Robby Ginepri.

OP must not have been watching tennis long.
Seeing 4 consecutive aces is really not all that rare, especially watching Isner, i've seen him do it at least 20+ times...

Ivanisevic served 4 aces in a row against Woodforde at 1992 Wimbledon. The game was only 26 seconds long. That might be the fastest game ever, I'm not sure.

Coria served 4 double faults in a row against Kiefer at 2006 Monte Carlo.

Dimitrov was close to DF all of his points in his service game in wimbledon this year against djokovic. He hit three double faults, even missed his first serve in the fourth serve but managed to hit a second serve in, embarresing if he lost the game by double faults.
