2 steps forehand 1 step back

I'm slowly discovering my tennis game is like my golf game — I got the basics down quite easily then plateaued. To progress further it's taken/ing alot of time and discipline... I think I'll be 80 and have finally figured it all out. I'll shed old habits and play very well for awhile, then develop new weird ones out of nowhere (does that happen to anyone else?). Currently, my forehand is doing this strange thing where I close the racquet head early sending the balls into the net. I never used to do this so it's bizarre to me, the best analogy I can give is it's very similar to a topsin forehand in table tennis:

I'm trying to be aware of it on my stroke, but it's sticking around... nothing terrible, just more often than I'd like. Any suggestions for a fix? Thanks!
 
Maybe your spacing and timing are off. Are you hitting out front?
Spacing I think is good, timing might be off. I'd like to think I'm hitting out front, maybe I'm not as much as I should be? The last outing I was noticing my wrist would turn over early on contact.
 
I do this when my mental image of the shot is off. I think I need to hit over the ball and manipulate it with my hands to get top spin. To get out of it I try to imagine just hitting thru the ball, concentrate on low to high hitting and stop ‘trying’ to put topspin on it. Concentrate on hitting out in front and hit thru and ignore the follow thru.
 
I do this when I'm falling slightly back and putting major spin on ball. The racquet really catches the ball nice but my momentum wrong way.
