Yeah it to me that the first part of the racket acceleration doesn't utilize the shoulder very well. It seems mostly a bit of racket drop due to the elbow bending.

I wonder if he maybe just has a really stiff shoulder.

Interesting, not sure I'm understanding what you mean. Are you referring to his racquet drop?

For me the biggest thing is that he drops his shoulders and upper arm aren't in a straight line. So you miss a lot of racket drop there already

are his injuries in any way serve related? i'm not sure why he changed his service motion
Not that I know of. At least his stance probably doesn't have anything to do with injuries — he's been experimenting back and forth with pinpoint and platform for several years, so he just seems really confused about it.

that seems like a really serious thing for a pro of his level to be confused about

he has noted serve specialist Michael Chang! (Chang's serve was not bad for his size though)

I'm talking about the image at 3.10 in the video, compared to Del Potro. I think that's the biggest thing, though the other things certainly contribute as well.

The pronation is definitely there, but in another direction, so he hits more on top of the ball, and I think that's having less of a throwing motion and more of a stretch arm and pronate motino. He doesn't throw it like a lasso so to speak.

Then there's definitely a lack of hip action, and I also think his motion isn't very explosive hear cause his knees are bent so deeply when he's moving to a pinpoint stance.

Right, his elbow is a bit low in the trophy pose. Yes, I agree it should probably preferably be a bit higher, though I don't think it's entirely unusual to have it a bit lower than parallell with the shoulders:

Not entirely unusual. But I'm pretty sure it's also not optimal. Back before the yips he had it a lot more straight and he definitely served bigger then.
