1-on the return of serve are you on your toes when hitting? I have a terrible ROS and realize I hit flat footed and jerk at the ball.

2-how to hit a good 2hbh return inside out? I always end up hitting it up the middle whenever trying to put some juice into it. Is it just a timing issue? I asked a few people and all I get is 'it's a hard shot, everyone has trouble with it'
 
Not actually on your toes, but rather on the "balls of your feet".

You should be executing a split-step as your opponent is hitting their serve (or is just about to contact the ball on their serve). I like to take a step forward (with either foot) as the server tosses the ball and then initiate the split-step just prior to contact (on their upward swing from the "scratch" position). Others prefer to start their split-step as the server contacts the ball (when they actually hear the contact). Whenever you decide is the best time for your split-step response, the important thing to to sync your "hop" to the serve of your opponent so that you can move/react in a timely manner to the ball coming off their strings.
 
I think of trying to hit from my heels with weight transfer forward onto the balls of my feet. If the ball is surprisingly fast, just the heels. If I have some time to get forward, from the balls of my feet.
I cannot maintain a stance on the balls of my feet. I need to push off using the my heels in order to get to the balls of my feet.
