ummm... good question !
at the US open, i'm not sure...

(with 4 tie-breaks there's, of course, the sampras-agassi of 2001, but it was a 4-setters)

outside of the US open, i can think of 3 matches that went to the 5th after 4 tie-breaks...
(i also can think of a player who played 3 "4 tie-breaks" matches in his career... but i think we already talked about it some years ago!)
