What was the score at the exact moment Nadal tweaked his back?
For me the match was won with break of serve at the start of the second when Stan ripped a one-handed backhand return winner. So when after this did the back start hurting?

I believe it was 3-6 0-2 (40-15)

I think it was 1-0, 0-30 or something like that when Nadal suddenly grabbed his back and bent over to stretch it. You could see the expression on his face change immediately. After he came back from the first time out, even the commentators mentioned that they'd never seen that kind of a look on his face when he was playing. And that he was "only 20%" because you could see he was just trying to spin his serves into the court after that.

I think later, when the painkillers kicked in, he could move a bit better for his groundies but he clearly wasn't trying to stretch too far for Stan's serves and his serve motion never did recover.
